Dear Mr. Yamada, This is in response to your e-mail regarding recent mistakes in your billing. I checked with our accounting department and learned that you are right. Recently, a number of invoices to your company were in error. This happened because of poor work by an inexperienced accounting clerk we hired several weeks ago. She misrepresented her ability and experience and we were lax in checking her background. The fault lies entirely with us. However we have terminated that person and taken steps to see that these kind of mistakes don’t happen in the future.
